Job Description

Job Summary:

Women & Children's Health Coordinator II is responsible to contributes to team coordination and member outcomes.

Essential Functions:

  • Act as a liaison and advocate of members and providers
  • Enhance member engagement and access to care by assisting member to integrate and optimize CareSource programs
  • Have an understanding of benefits and assist in educating members of benefits as appropriate
  • Enhance effectiveness of member care by acting as a resource of knowledge concerning women and children's health, care and concepts, plan benefits, and options for access to care
  • Initiate outreach contact with member to assist with coordinating care, continued enrollment/eligibility and follow up appointments
  • Appropriately solicit health information from members through telephonic interviewing as directed for specific initiatives and projects
  • Facilitate and coordinate services based specific initiative, focus developed in collaboration with all stakeholders
  • Establish positive communications with members and providers via telephonic contact
  • Assist team when requested, with tracking, compiling data, and computing needs
  • Maintain current, timely, and accurate documentation within the clinical platform-Guiding Care following protocols and guidelines of department
  • Actively participates in team meetings
  • Develop and maintain knowledge of the company's business and regulatory environments
  • Facilitates and participates in coordination of community events.
  • Point of contact and attends community events as needed.
  • Develops process and tracking of outcomes for community events and member outcomes
  • Perform any other job duties as requested

Education and Experience:

  • High School Diploma or General Education Diploma (GED), is required
  • Previous experience in health-related fields is preferred

Competencies, Knowledge and Skills:

  • Basic proficiency with Microsoft Office Suite to include Outlook, Word and Excel
  • Sensitivity to and experience working within different cultures
  • Comfortable and interested in working with members around issues of healthcare access and adherence
  • Appropriate language proficiency
  • Familiarity with healthcare field
  • Excellent understanding of local and community resources
  • Excellent interpersonal and telephonic skills
  • Ability to work within a team environment, manage time and multitask
  • Ability to work in compassionate, supportive, and collaborative ways with members and families
  • Self-motivated and inquisitive
  • Comfort with asking pertinent questions

Licensure and Certification:

  • To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 - March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions:

  • General office environment; may be required to sit or stand for extended periods of time
  • Travel may be required-up to 50% or more

Compensation Range:
$34,500.00 - $55,100.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. We are highly invested in every employee's total well-being and offer a substantial and comprehensive total rewards package.
